Sample Orders

To order samples by email or fax, please provide the following information:

• Billing and shipping address
• Contact name, phone number and email address
• Model number and quantity requested
• UPS or Fedex shipping account number
• Samples & blank goods will be billed at EQP price plus freight.  No minimum quantity is required and usually ships within 48 hours.
• Samples are not returnable and invoices are due upon receipt.
